Housing With Services at Cedar Sinai Park

In addition to Kehillah Housing and the new nursing home, Cedar Sinai Park is actively working to develop a new community service model that will provide care and services for seniors in the comfort of their own homes. This week we’ve taken the first step in developing the program with the Housing With Services Summit in Salem, Oregon.
As reported in the Fall 2011 issue of our newsletter “Generation to Generation”, we received seed funding from the Jewish Federation of Greater Portland‘s Community Impact Grant Fund. The program will support seniors in safely aging at home through access to in-home care, care management, Jewish cultural activities, rehabilitation, transportation, and health and wellness education.
We’ll develop this wrap-around service model over two years, in two phases: 1) Planning and Needs Assessment, and 2) Implementation. The first phase began this Monday, November 28th, at a Housing With Services Summit in Salem, Oregon, coordinated by Cedar Sinai Park and the Oregon Department of Human Services.
DHS Director Erin Kelley-Siel and Oregon Health Authority Director Bruce Goldberg both expressed support for the project. Dr. Robyn Stone from the Leading Age Institute for Aging Services presented an overview of the housing-with-services model. She pointed out that housing with services is now an evidence-based public health model that uses housing as a platform to promote prevention and early intervention services. Nancy Eldridge, CEO of Vermont-based Cathedral Square, offered a case study of a successfully implemented program in her state that we’ll be learning a lot from.
David Fuks then walked the audience through the initial plan for developing the program (see the embedded slideshow below). The afternoon sessions revolved around ways to collaborate and implement the program.
